Seven Counties Services is the point of entry
for its region for First Steps, the Ky. Early Intervention System.
Services include: developmental evaluation, referral to community
providers of early intervention services, development of an Individualized
Family Services Plan, and primary service coordination. For more

Fee: Evaluation, assessment and service coordination are
at no cost to families. Other intervention service may require a
monthly Family Sharefee based on household income. Administered
by the Ky. Cabinet for Health Services Department for Public Health.
Eligibility: Early intervention: Children birth through 2
years with a developmental delay or particular medical condition
known to cause developmental delay. Primary service coordination:
First Steps clients who choose Seven Counties Services as their
primary service coordination provider.
Referral: Self-referrals accepted as well as from pediatricians
and other physicians or health and human service providers. Call

Hart
Supported Living Program

This program involves individualized supports
designed and coordinated by individuals with disabilities (or their
designee) to help provide independent living in their homes and
participation in the community. Funds are awarded based on a grant
application.
Fee: No cost to participate. Costs of program are underwritten
by the Ky. Department for Mental Health/Mental Retardation Services.
Eligibility: Persons who meet criteria of the Americans with
Disabilities Act definition of disability.
Referral: Self-referrals accepted as well as those from other

Department for Mental Health/Mental Retardation (DMHMR)
Residential Program

Residential services are provided through an agreement
with a number of community providers.
These services include developing an alternative family placement
for an individual, support, and monitoring of placement.
Fee: No cost to individual. Costs of this program are underwritten
DMHMR.
Eligibility: Children and adults diagnosed with mental retardation
or other developmental disability and who are not receiving funding
for these services though another program.
Referral: Self-referrals accepted as well as those from other

Supports
for Community Living Waiver
Robyn Thomas, unit manager
This is a home and community-based waiver under
the Kentucky Medicaid program and was developed for Kentucky residents
as an alternative to institutional care for an individual with mental
retardation or developmental disabilities. SCL allows an individual
to remain in or return to the community. Services include: case
management; residential supports; adult day training; supported
employment; community living supports, behavior supports, psychological
services; occupational therapy; physical therapy; speech therapy;
respite; and specialized medical equipment and supplies.
Fee: Medicaid coverage only.
Eligibility: Persons with mental retardation or other developmental
disabilities meeting Ky. Medicaid criteria for ICF/MR/DD, and who
meet other Medicaid requirements.
Referral: Self-referrals accepted as well as from other health
and human service providers.
Potential clients currently are placed on an extensive waiting list.

The Networks program provides adult day habilitation
on-site and in a variety of community settings. Training focuses
on skill enhancement. Job profiling, development and coaching also
is available to some program participants.
Fee: Monthly activity fee. Program is underwritten by the
Ky. Department for Mental Health/Mental Retardation Services.
Eligibility: Persons must be a Supported Community Living
(SCL) waiver recipient.
Referral: Accepted from individuals or families of individuals
in Seven Counties' region who are SCL waiver recipients.

Support

Training or other assistance to individuals who
live in their own home or family homes. These supports include a
variety of activities such as shopping, daily living skills household
care.
Fee: No cost to client. Services are underwritten by the
Ky. Department for Mental Health/Mental Retardation Services.
Eligibility: Individuals diagnosed with mental retardation
or other developmental disabilities and not receiving funding for
these services through another program.
Referral: Self-referrals accepted, as well as from families
and other health and human service providers within Seven Counties

Support Services

This program is the regional point of entry for
persons with developmental disabilities in the Seven Counties region.
The staff provides information, resources and referral linkages,
services coordination, crisis response and prevention, and long-term
planning for individuals and families affected by developmental
disabilities.
Fee: No cost to individuals and families. Services are underwritten
by the Ky. Department for Mental Health/Mental Retardation Services.
Eligibility: Persons 3 years and older with developmental
disabilities who are at risk for involvement with a number of human
service systems, who need support during crises, and who need assistance
with linkage to appropriate supports and services.
Referral: Accepted from individuals and families in Seven
Counties region who meet MR/DD eligibility criteria, and from
health, mental health, or other human services professionals. Please

This service involves screening and evaluation
of persons with a mental illness and/or mental retardation to determine
appropriateness for nursing home placement. Provides services mandated
by legislation.
Fee: No cost to participants. Costs are underwritten by the
Ky. Dept. for Mental Health/Mental Retardation Services.
Eligibility: Accepts adults referred to licensed nursing
facilities and having a diagnosis of mental illness and/or mental
retardation; have functional impairments/limitations in major life
activities due to mental illness; have received recent treatment
for mental illness or mental retardation.
Referral: Referrals taken from long-term care facilities
and other health and human service providers. To refer, call coordinator

WHAS Crusade for
Children
Lending Library
of Adaptive Equipment

This program loans adaptive equipment, such as
special chairs for children, walkers, computerized language systems,
and therapy balls, to enhance normal physical development and the
acquisition of functional communication skills. Equipment can be
used as long as needed and then returned to Seven Counties.
Fee: No cost to borrowers. Program is underwritten by the
Ky. Department for Mental Health/Mental Retardation Services and
WHAS Crusade for Children.
Eligibility: Children/youth age birth to 19 with developmental
disabilities.
Referral: Family self-referrals accepted, as well as those
from occupational, speech, and physical therapists, physicians,

Acquired
Brain Injury (ABI) Case Management Services
Bonnie Thorson-Young, division director
ABI case managers are resopnsible for initiation,
coordination, implementation and monitoring of the assessment, evaluation,
intake and eligibility process for ABI Waiver services. This includes
assisting the individual in obtaining services and monitoring the
delivery of those services. Case managers also assist an ABI recipient
in the identification, coordination and facilitation of the interdisciplinary
team and interdisciplinary team meetings related to developing and
updating the plan of care.
Fee: Medicaid eligibility.
Eligibility: Adults aged 21-65 years who meet nursing facility
level of care, have a primary diagnosis of acquired brain injury,
have potential for rehabilitation and reintegration into the community
and who are Medicaid eligible.
Referral: Call Developmental Services at 459-5292 and ask
for the family support services intake coordinator.
Targeted
Case Management: For children ages 3 to 21
Vicki Evans, Principle Case Manager
This is a Medicaid funded program to meet individual
needs of children and adolescents who have a mental health diagnosis
as well as mental retardation or other developmental disability.
This program provides intensive service coordination. Each child
has an individualized service plan, which may include after school
and summer programs, therapeutic supports, behavior intervention,
day treatment, school advocacy and respite.
Fee: No cost to families. Eligible individuals receive funding
through Medicaid.
Eligibility: Medicaid eligible; under age 21; documented
mental health and developmental disabilities.
Referral: Clients/families, mental health or other health
or human services professional.

SPEAK is an association offering supports to Direct
Support Professionals (DSPs) in the Metro Louisville region. DSPs
provide hands-on care to a wide range of individuals with disabilities.
SPEAK offers pre-service orientation, a mentoring/apprenticeship
program; monetary recognition; training and discussion opportunities.
Fee: No cost to DSPs. Funded primarily by a three year grant
through the U.S. Centers for Medicare and Medicaid Services. Additional
funding from partner agencies, including The Council on Mental Retardation,
Cedar Lake Residences, Community Living, Inc., Dayspring, Dreams
with Wing, Exceptional Teens & Adults, Louisville Diversified
Services, and the Mattingly Center.
Eligibility: Membership open to DSPs with at least one year
of service with one of the partner agencies listed above. Events
are open to others for a nominal fee.
Adult Targeted Case Management

This is a Medicaid funded program to meet the
individual needs of adults (18 years or older) who have a chronic
mental illness as well as mental retardation or other developmental
disability. This program provides intensive service coordination.
Participants have an individualized service plan in conjunction
with a mental health treatment plan, overseen by a mental health
therapist.
Fee: Medicaid
Eligibility: Adults aged 18-65 who have a chronic mental
illness as well as mental retardqation or other developmental disability.

Crisis Prevention and
Response

Individualized service coordination is provided
on a case-by-case basis at whatever frequency and intensity is necessary
to restore security and continuity for adults in crisis. Individualized
vouchers are available to purchase appropriate supports and services,
including non-traditional purchases that may have the potential
to fortify natural supports.
Fee: No cost to recipients. Underwritten by the Departmenta
for Mental Health & Mental Retardation
Eligibility: Residents of the SCS region who have a diagnosis
of mental retardation or developmental disability who are experiencing
a behavioral or psychiatric crisis.

Services intake coordinator.
PASRR Specialized Services
Martha Casper, Principle Case Manager
Supports and services to individuals residing
in nursing facilities who have a diagnosis of mental retardation
or other developmental disability and have been identified through
the PASSR evaluation process as needing specialized services. Services
are based on person centered plans and may include community coaching,
day programming, and adaptive equipment/assistive technology.
Fee: No cost to recipients. Underwritten by the Department
for Mental Health & Mental regardation.
Eligibility: The Division of Mental Health & Mental Retardation
only.
