Adult Mental Health Services
Seven Counties Services’ adult mental health programs offer an array of services for persons with mental illness, such as schizophrenia and bi-polar disorders. Persons with these illnesses often have a history of multiple hospitalizations and benefit from assistance to achieve recovery in the community.
Jean Marlatt Center(s) for Supported Living
The mission of the Centers for Supported Living is to promote the recovery, self-sufficiency and empowerment of adults living with mental illness. To this end, interventions are based on a belief that a comprehensive, coordinated continuum of treatment opportunities has the greatest probability of achieving the mission of the program. This approach is based on a holistic approach to meeting peoples' needs, with a specific focus on helping people to maximize independent functioning through stable community living, socialization, employment, symptom management, self-advocacy, education, and improved self-esteem. The employment of Peer Support Specialists and the use of person-centered planning throughout the division helps to promote the transformation to a recovery-based model and to facilitate the achievement of the division's mission. Fee: Accepts Medicaid and Medicare. Eligibility: Meets diagnostic criteria for having a mental illness. For Case Management and Peer Support Services, meets state defination for having a severe mental illness. Criminal Justice/Jail Diversion Services

This program intervenes in the court system in Jefferson County on behalf of persons with a severe and persistent mental illness to help ensure the court’s appropriate treatment of persons with mental illness and to improve follow-up and care. Services include assessment; coordination of information exchanged between the court system and Seven Counties, development of alternative sentencing plans and jail diversion; consultation with judges and attorneys, and limited case management. Fee: No cost to participants. Funded through a grant from Louisville Metro Government. Eligibility: Accepts persons diagnosed with a severe mental illness and in jail facing a court appearance in Jefferson County. Referral: Accepted only from judges, officers of the court, attorneys or other representatives of the criminal justice system. Therapeutic Rehabilitation Clubhouses
Seven Counties’ three therapeutic clubhouses are for persons with a severe mental illness, such as schizophrenia or bi-polar disorder. The clubhouses revolve around "work-ordered days." Members complete tasks essential to daily clubhouse operations that help them assess and strengthen their skills. Members measure progress by reductions in hospitalizations, enhanced daily living skills, and increased independence. Members participate in educational, clerical, kitchen, maintenance, business, and social/recreational activities. Fee: No cost to participants. Accepts Medicaid. Costs also underwritten by the Ky. Dept. for Mental Health/Mental Retardation Services. Eligibility: Accepts Seven Counties’ clients 18 years and older with a diagnosis of severe mental illness. Clubhouse membership is voluntary. Referral: Self-referrals accepted as well those from family members and Seven Counties’ staff. To make a referral, contact a clubhouse manager at the numbers below.

Vocational Alternatives

Vocational Alternatives provides supported employment services for adults with severe and persistent mental illness and dual diagnosis. Services include employment readiness skills education, job placement, job coaching, long-term individual and group peer support. Fee: No cost to participants. This program is underwritten by the Ky. Dept. for Mental Health/Mental Retardation Services and the Ky. Dept. of Vocational Rehabilitation. Eligibility: Accepts adults receiving treatment for severe mental illness. Eligible individuals may also have intellectual and/or developmental disabilities and/or a history of substance abuse. Referral: Self-referrals accepted as well as referrals from family members, human and medical service providers, schools, etc. To make a referral, contact the unit manager at above number.
The Center for Rehabilitation and Recovery

The Center for Rehabilitation and Recovery is a 38-bed licensed personal care home that provides mental health treatment and psychiatric rehabilitation services based on a recovery model to adults with psychiatric disabilities who have had unsuccessful community living experiences. It was jointly developed by the Ky. Dept. for Mental Health/Mental Retardation Services (DMHMRS), Central State Hospital, and Seven Counties. The center provides basic health and health-related services; medication management; individualized rehabilitation and mental health treatment; skill-building classes and educational groups; individual and group therapy; social and recreational activities; pastoral care services; and educational and vocational services. Fee: No cost to residents. Costs are underwritten by DMHMRS. Eligibility: Individuals with a diagnosis of severe and persistent mental illness and repeated hospitalizations with limited success living independently in the community. Referral: Referrals must be through Seven Counties (502) 253-7500 or Central State Hospital. An interagency committee that includes representatives from the Center, DMHMRS, Adult Protective Services, Central State Hospital, and mental health consumers, screens all referrals. Residents of Kentucky state hospitals receive priority placement. Currently, clients seeking admission are placed on a waiting list for admission.
Homeless Outreach Team

This project, a joint effort with the Louisville Metro Coalition for the Homeless and Projects for Assistance in Transition from Homelessness, helps engage people who have a mental illness and who are homeless. Case managers encourage and assist them in accessing treatment and support services, reaching out in shelters and elsewhere to establish relationships. The goal is to help individuals remove themselves from the rolls of persons who are homeless and access mental health services. Fee: No cost to participants. Funded through federal grants from the U.S. Dept. of Housing and Urban Development (HUD) and Projects for Assistance in Transition from Homelessness (PATH). Eligibility: Persons who are homeless and presenting with symptoms of mental illness. Referral: Referrals accepted from shelters, other human and medical service providers, local business owners, advocates and private citizens. Contact the supervisor at 589-8600 ext. 1280.
Pre-Admission Screening and Resident Review (PASRR)

This service involves screening and evaluation of persons with a mental illness and/or intellectual and/or developmental disabilities to determine appropriateness for nursing home placement. Provides services mandated by legislation. Fee: No cost to participants. Costs are underwritten by the Ky. Dept. for Mental Health/Mental Retardation Services. Eligibility: Accepts adults referred to licensed nursing facilities and having a diagnosis of mental illness and/or intellectual disability; have functional impairments/limitations in major life activities due to mental illness; have received recent treatment for mental illness or intellectual disabilities. Referral: Referrals taken from long-term care facilities and other health and human service providers. To refer, call coordinator at (502) 585-2008.
Services for the Deaf and Hard of Hearing

Staff skilled in American Sign Language provide mental health services to adults and children with hearing loss and their family members. Services include individual, family, and group therapy as well as in-school group and individual treatment. Most services are provided in American sign language; certified, licensed interpreters are used for psychiatric, medical and some other services. Limited case management/collateral services also are available. Clients can be seen at the West Broadway location or other arrangements can be made.
Staff also provide education, outreach and consultation on mental health issues in the community. Workshops are available, and can be tailored to audience, regarding accessibility, mental health interpreting in mental health settings, and other topics. Fee: Accepts Medicaid, Medicare, some private insurance. State funding is also available for the uninsured. Sliding fee schedule available. Eligibility: Individuals who are Deaf or Hard of Hearing and their family members. Referral: Accepts self-referral, referrals from other health professionals, and referrals from agencies serving the Deaf and Hard of Hearing communities.
Traumatic or Acquired Brain Injury Services

This program provides treatment and care of persons with traumatic or acquired brain injury (TBI/ABI). Fee: Accepts Medicaid and Medicare Eligibility: Persons 18 and older with TBI/ABI; those with TBI/ABI and mental illness.
